Thank you for completing the wellspring education registration form.

Your form has been successfully submitted and we will contact you after reviewing your submission.

Students need to provide copies of the following updated documents(if already provided students don’t need to resubmit):

• a. Social Security card,
  
  b. Birth Certificate,
  
  c. Document that provides proof of address, such as driver's license, mail, paystubs, bank statements, etc. ,
  
  d. Current IEP or 504 plan,
  
  e. and a Letter of Withdrawal (if available).

Please email copies to: olivia@wellspringmultiservice.org or bring with them to Orientation on the first day of classes. If you need transportation or babysitting services we will contact you to make final arrangements.
